    MyPathway Clinical Portal

The MyPathway Clinical Portal delivers a personalised, digital interaction between patients and clinicians, supporting clinicians to monitor patients in real time into a single source of truth.

    Why MyPathway Clinical Portal?

    Reduce hospital appointments

Clinicians are able to assess risk and priorities for intervention, without patients having to attend an appointment

    Improve the patient experience

Enable clinicians to share information and deliver care to patients within the comfort and safety of their own homes

    Support digital triage processes

Digital triage enables clinicians to make quick and accurate treatment referrals to the recommended healthcare professional

    Reduce administrative burden

Contact large cohorts of patients quickly analyse large amounts of data more efficiently

    Support PIFU targets

Reduce waiting lists by enabling patients to request a follow-up appointment (PIFU) via the MyPathway Patient App

    Reduce DNA rate

Reduce DNA (Did Not Attend) rate by providing patients with correct, up to date information

    Support NHS Net Zero targets

Reduce the environmental and financial impact of paper processes

Extending the use of MyPathway trust-wide will help us better support patients with long-term conditions much more proactively to manage their care than via face-to-face appointments alone. They can also undertake remote monitoring of patients and deliver flexible and timely communications to patients directly via the dashboard.

The MyPathway Clinical Portal combines operational breadth and clinical depth, to offer hospitals a single digital solution for all patient engagement, communication and analytics across multiple clinical pathways.

    Monitor patients in real time

The award-winning MyPathway Clinical Portal enables clinicians to monitor and communicate with patients in real time, across multiple clinical pathways. The intuitive dashboard provides enriched patient data to support enhanced decision-making, whilst reducing clinical administration to release time and enable clinicians to focus on patients requiring immediate care. MyPathway is often used to improve patient interactions during treatment for long-term conditions, in areas including chronic pain, musculoskeletal, motor neurone disease and rheumatological conditions. MyPathway also supports effective communications in shorter pathways (shorter-term conditions), such as physiotherapy or orthopaedic surgery.

    Manage large patient cohorts efficiently

The MyPathway Clinical Portal dashboard provides a single source of truth, transforming how clinicians can manage their increasing caseloads. Real-time graphs of key markers enable the care team to identify patient health trends over time and intervene if required. MyPathway’s operational automation provides patient data without an administrative need to request or process it manually and supports clinicians in managing large patient cohorts efficiently and safely. Clinicians can automatically share educational resources to minimise unnecessary hospital appointments, releasing clinical time for vital face-to-face appointments. Staff can also create a library of predefined automated messages or questionnaires, to send alongside bespoke communications to patients via the app, helping to reduce administrative time and maximise clinical time with patients.

    Release clinical time

Automated, integrated pathways release clinical time. By integrating with the hospital’s PAS for events in the patient’s journey, messages, questionnaires, and resources are sent automatically at the right moment. Advanced search, filter, and bulk operations simplify the management of large cohorts of patients. Clinicians can also effectively manage their waiting lists through Patient Initiated Follow-Ups (PIFU), empowering patients to initiate appointments when required. Data can be provided at patient, pathway, service, and population levels to support strategic decision-making and service improvements based on fine-grained data gathered directly from patients.

What makes MyPathway Clinical Portal different?

A service and system agnostic portal enabling clinicians to track and support remote patient pathways in real-time. Graph views of key markers enables the care team to identify trends over time and intervene if required.
