It’s well documented that longer stays in hospital can lead to worse outcomes for patients. Ensuring patients are at the right level of care is essential to avoid deconditioning and to reserve beds for those who need them most. The MCAP clinical utilisation review software supports hospitals to tackle patient flow blockages, delayed transfers of care and stranded patients by identifying opportunities to reduce Length of Stay. MCAP can also be offered as a one-off snapshot review, or alternatively as an ongoing managed service, delivering regular, updated data. When the MCAP challenge criteria is integrated into a patient flow module, it will identify twice the number of patients that are able to move to alternative care pathways than previous Medically Fit For Discharge (MFFD) and Fit For Discharge Therapy (TFFD) processes.
The MCAP solution includes a clinically based set of criteria combined with a data-gathering and reporting system for use in a range of specialities, including medical, surgical and mental health. It is used to place patients for admission or continued stay at the most appropriate level of care given their individual care service requirements. The data is collected at patient level and aggregated to give a strategic view across trusts, providing an objective and standardised methodology to measure reasons to reside. Only one criterion needs to be met for an admission or continued stay to be considered medically necessary.
MCAP has worked with NHS England and NHS Improvement to embed their mandated daily review alongside the Royal Colleges of Medicine endorsed “Reasons to Reside” approach and automatically generate all NHSE Sitreps, saving time for staff and ensuring data is consistent and accurate. MCAP can be completely integrated with current hospital systems, so whilst the clinical review is completed against its unique criteria, the inputs and outputs flow seamlessly to existing systems as required, ensuring the availability of the data to all levels of management within any Integrated Care System, avoiding data duplication or siloed data. The data can also be accessed remotely via a secure link, supporting staff to work remotely.
