NHSE funding to support COPD patients this winter

Target Funding is available to support the introduction of tech-enabled services, such as MyPathway, to support patients at the greatest risk of admission from their COPD.

Winter poses significant challenges for patients with Chronic Obstructive Pulmonary Disease (COPD), a condition characterised by persistent respiratory symptoms and airflow limitations. Cold weather and seasonal infections like influenza can exacerbate COPD symptoms, leading to increased emergency admissions.

In its COPD Winter Approach, NHS England is inviting providers to bid for Target Funding, to encourage the expansion of the remote monitoring model across the NHS in England; sites already remote monitoring for their COPD patients at scale are reporting a 40-60% reduction in emergency admissions.

In a letter issued on the 26th of July 2024, COPD Winter Approach: 2024/25 Call for Applications for Targeted Funding, The Cardiovascular Disease and Respiratory Programme and the Primary Care and Community Services Delivery Unit outline their intent to elevate practical and proactive approaches to identifying people with COPD most at risk of exacerbation this winter, as well as strengthen the support for those identified. Integrated Care Boards have until the 19th of August 2024 to bid for £125k to facilitate the rapid introduction of a tech-enabled remote monitoring service, capable of supporting patients who are vulnerable to admission from their COPD.

VitalHub UK’s MyPathway solution offers a proactive approach to managing COPD, aiming to reduce these admissions and improve patient outcomes during the critical winter months.

    • Proactive Monitoring and Early Intervention: MyPathway enables healthcare providers to monitor COPD patients remotely and continuously, allowing for real-time data collection on symptoms and overall health status. By tracking key indicators such as breathlessness, oxygen saturation, and exacerbation frequency, MyPathway can help identify when a patient’s condition is deteriorating. Early detection of worsening symptoms allows healthcare teams to intervene promptly, potentially preventing the need for emergency care.
    • Personalised Care Plans: COPD is a complex condition that varies significantly from patient to patient. MyPathway allows for the creation of personalised care plans that are tailored to each patient’s specific needs. These plans include exercise regimens, and self-care strategies that patients can follow at home. By empowering patients to manage their condition more effectively, MyPathway helps reduce the likelihood of hospital admissions due to complications or exacerbations.
    • Patient Engagement and Education: A key feature of MyPathway is its ability to engage patients in their own care. The platform provides educational resources that help patients understand their condition, recognise early warning signs, and take appropriate action to manage symptoms. By improving patient knowledge and confidence, MyPathway encourages adherence to treatment plans and promotes healthier lifestyle choices, which are critical factors in avoiding emergency admissions during winter.
    • Enhanced Communication with Healthcare Providers: MyPathway facilitates seamless communication between patients and healthcare providers. Patients can report their symptoms, ask questions, and receive advice without needing to visit a clinic or hospital. This ongoing communication ensures that patients feel supported and can receive timely guidance, reducing the likelihood of their condition worsening to the point of requiring emergency care.
    • Data-Driven Decision Making: The data collected through MyPathway provides healthcare providers with valuable insights into the health of their COPD patients. This data can be used to identify trends, predict potential exacerbations, and allocate resources more effectively. By leveraging this data, healthcare providers can make informed decisions that improve patient care and reduce the strain on emergency services during the winter months.

VitalHub UK’s MyPathway solution is a powerful tool for managing COPD patients, particularly during the challenging winter season. By enabling proactive monitoring, personalised care, patient engagement, and enhanced communication, MyPathway supports admission avoidance and helps ensure that COPD patients receive the right care at the right time. As we head into winter, solutions like MyPathway are essential for reducing emergency admissions, improving patient outcomes, and easing the burden on healthcare systems.
